چکیده

To assess the efficacy of novel cardioprotective therapies for reducing myocardial infarct size, it is important to accurately measure the area at risk, as this is needed to assess myocardial salvage, a more sensitive measure of cardioprotection. Although T2-weighted cardiovascular magnetic resonance (CMR) imaging is a promising technique for quantifying the area at risk, this approach does have its limitations. With the availability of hybrid positron emission tomography–MR (PET-MR) imaging, we now have the unique opportunity to simultaneously combine tissue characterization by CMR imaging with the metabolic insights from PET. Two cases are presented to highlight the potential utility of hybrid PET-MR imaging in acute reperfused ST-segment elevation myocardial infarction in investigating the changes in cardiac metabolism in the area at risk and in the assessment of myocardial salvage. Hybrid PET/MR metabolic imaging of the reperfused infarct - new biology, future directions

Background Multi-modality interrogation of myocardial health and disease using fused Positron Emission Tomography (PET) and magnetic resonance (MR) data has been limited by the need for separate scans, and complex and potentially imprecise co-registration of the individuallyacquired PET and MR images. We sought to explore the potential of hybrid simultaneous PET/MR (hs-PET/ MR) imaging to preci...

Measuring myocardial salvage: hormone cardioprotection

Ischemic heart disease (IHD) is the leading cause of death and disability worldwide. For patients presenting with an acute ST-segment elevation myocardial infarction (STEMI) the treatment of choice is timely reperfusion by primary percutaneous coronary intervention (PPCI). However, the process of reperfusion can in itself independently induce myocardial injury and cardiomyocyte death—a phenomen...

Sex differences in myocardial salvage and clinical outcome in patients with acute reperfused ST-elevation myocardial infarction: advances in cardiovascular imaging.

BACKGROUND There is conflicting evidence regarding sex-based differences in myocardial salvage and clinical outcome in patients after ST-elevation myocardial infarction (STEMI). The aim of this study was to investigate whether there are sex-associated differences in infarct characteristics (myocardial salvage, infarct size, microvascular obstruction) and clinical outcome in STEMI patients who a...

Left Ventricular Thrombus Formation After ST-Segment-Elevation Myocardial Infarction: Insights From a Cardiac Magnetic Resonance Multicenter Study.

BACKGROUND Data on left ventricular (LV) thrombus formation after primary percutaneous coronary intervention for ST-segment-elevation myocardial infarction (STEMI) are scarce. The aims of this study were to assess the (1) incidence of LV thrombi using cardiac magnetic resonance in a multicenter cohort of STEMI patients and (2) prognostic relevance of LV thrombi at 1-year follow-up. METHODS AN...

Which cardiac magnetic resonance parameters predict outcome in acutely reperfused non-ST-elevation myocardial infarction?

Background Clinical outcome after acute ST-elevation myocardial infarction (STEMI) has been shown to be predictable by myocardial salvage index (MSI) and microvascular obstruction as determined by cardiac magnetic resonance imaging (CMR). In the setting of non-ST-elevation myocardial infarction (NSTEMI) prognostic significance is less well investigated. Due to comparable incidence of major adve...
